Do you accept new patients and referrals?
Yes, and thank you for your confidence. We appreciate your referrals and promise to extend the same comprehensive, caring service to your friends and coworkers that we give you every time you visit. We even offer a referral rewards program where you can earn gift cards to area businesses for every referral you make.
What payment options do you offer?
We accept cash, check, and debit or credit cards (MasterCard/Visa/Discover). If you are covered by a dental insurance plan, we will work with you to help you get whatever benefits you are entitled to. Do I need to arrive early for my first appointment?
Please be sure to request a prescription prior to your appointment, or if you are unsure, contact us and we can help.
What do I need to bring to my first appointment?

Please bring the following items with you to your appointment:

  • Patient Information Form
  • Dental Insurance Card (if applicable)
  • Identification such as Driver’s License, Military ID or State ID
  • Patient Health History Form
  • HIPAA Consent Form
  • Patient Authorization Form
How long will my first appointment last?

It varies, but please plan on 1 to 1.5 hours for the first visit.
